Introduction
HNYMPC recognizes the complexity and interrelationship between health conditions and functional limitations. We are a medical group that works in concert with a multi-disciplinary health care team that includes the patients primary care physician, attending specialty physicians and of course the patient and/or the patients primary caregiver to coordinate the health care of certain Medicare, Medicaid, and Commercial managed care members to perform annual wellness assessments, offer priority care management to patients with chronic medical conditions, and transition of care management. This high-touch coordination occurs along a continuum from social to medical in range of settings that includes the patients home, the primary care physicians office and HNYMPC medical offices.
